With or without insurance, we can arrange a payment plan to meet your needs.
We offer extended payment plans with prior credit approval through
appropriate lending institutions.
As a courtesy to other patients who need our services,
please give 48
hours notice
if you must cancel an appointment.
Thank you! Our policy is to charge for missed appointments
at the rate of a normal office visit if 48 hours notice is not given.
Please help us serve you better by keeping your scheduled
appointments.

The full balance is your responsibility whether your insurance company
pays us or not. If you insurance company has not paid your account
in full within 45 days, payment in full of the balance will be your
responsibility. Please be aware that some, and perhaps all, of the
services provided may be non-covered services and not considered
reasonable and necessary under your dental insurance program.
Insurance Payments Involving a Participating Agreement:
We accept assignments of insurance benefits and participate with
many insurance companies. All co-pays and deductibles are due
prior to treatment. However, we cannot bill your insurance company
unless you give us your insurance information and an original claim
form. Your insurance policy is a contract between you and your
insurance company. We are not a party to that contract. When we
accept assignment of benefits, we require you to be pre-approved on
our extended payment plan, or provide a credit card with authorization
to bill that account for the balance.
Insurance Payments Not Involving a Participating Agreement:
When we do not accept assignment of benefits, your insurance
company directly pays you. You are responsible for payment at
each appointment, just as though you had no insurance coverage.
You need to provide us with all of the information so we can
properly process your claims for you.
Usual and Customary Rates:
Our practice is committed to providing the best treatment for our
patients, and we charge what is usual and customary for our area.
You are responsible for payment regardless of any insurance
company's own definition and determination of usual and
customary rates.
Service Charges:
A 2% finance charge (24% annually) may be added to any
second billing or balance over 60 days.
